Your Next Front-Desk Hire Might Not Be a Person

Here's a number worth sitting with: 69% of patients say they'd switch providers over poor communication. Not bad care. Bad communication. A missed call, a full voicemail box, a Monday morning where the front desk spends two hours returning calls instead of checking patients in.

This week, three things happened that tell me the fix is no longer theoretical.

GoTo Connect built AI scheduling directly into athenaOne. A patient calls at 9pm, the AI books, reschedules, or cancels, and it lands straight in the EHR. No voicemail. No "we'll call you back tomorrow." No sticky note on someone's monitor.

Meanwhile, UnitedHealthcare and Aetna are deploying AI agents that call your office to book appointments on behalf of their members. Read that again. The AI isn't just answering your phones anymore. It's calling you. If your scheduling workflow can't handle a caller that never gets tired, never mishears, and never waits on hold, that's worth solving before it becomes your Monday.

And Heidi, the ambient documentation company, just raised $340 million to push beyond scribing into agentic workflows. They're already handling around 2.8 million patient interactions a week.

So what's the honest read?

The technology works. The good ones genuinely handle routine scheduling calls well. They're not replacing your front desk. They're absorbing the calls your front desk never wanted: the 7pm reschedules, the "what are your hours" calls, the appointment confirmations.

But here's what I'd tell any practice owner before buying: the EHR integration is everything. An AI agent that books into a spreadsheet or emails you a summary is just a fancier voicemail. The one worth paying for writes directly into your scheduling system, logs every call, and hands off to a human cleanly when the conversation goes sideways.

Three questions for any vendor

  1. Does it write directly into our EHR, or does someone retype it?
  2. What happens when a patient asks something it can't handle?
  3. Show me the call log. Every call, recorded and reviewable.

The practices that figure this out early won't just miss fewer calls. They'll stop losing patients they never knew they were losing.
